To provide support and coordination of patient care for specific neurological conditions as required by department and support for Clinic operation needs.
Job Responsibilities
You will be responsible for the following:
Assist in coordinating care for patients with Specific Neurological conditions:
Appointments
- Booking of follow-up appointments for specific neurology patients.
- No show follow-up and coordination of new rescheduling of appointment for patient if required.
- Assist in rescheduling of specific appointment upon request and relevant pre-appointment investigations.
Investigation
- Ensure patient’s investigations (bloods/imaging) are ordered and done.
- Ensure patients are well informed of their investigation appointments.
- Liaison person between nurse and Doctor on patient's enquiry
Operations & Case Management
- Discuss with Neurologist to identify suitable patients for referral to our external partners for continuous patient care management. eg: AIC
- Assist Doctor on completion of referral forms to external partners.
- Facilitate patient’s transfer to external care and provide relevant support to patient/family members.
Admission
- Coordination of admission date with doctor and patient.
- Perform routine or force-booking of bed for admission cases.
- Conduct financial counselling for admission cases.
- Remind and ensure patient to turn up for admission.
- Documentation of encounter for admission
- Liaison for patient between clinic and PSC on the needs for patient’s enquiry or changed of admission date when required.
Data Collection
- Collection and submission of data to monitor and evaluated service outcome.
Clinical duties
- Conduct memory test and other neurology assessment
Clinic operations
- Assist in running clinic sessions if required.
- Triage direct access referrals in Clinic EPIC’s escalated work queue
- Escalate to triage Doctors in the event of long wait time for urgent referrals.
Care Management
- Monitoring of first visit wait time in a timely manner.
Perform any other duties as assigned by the reporting officer.
