The case management officer will always stay in close communication with internal and external stakeholders as well as patient across the care continuum to ensure care coordination, efficient care delivery, and removal of care barrier. S/he will coordinate and assist clinical and nursing team in the planning and logistic needs of the patients in the pretransplant setting and is also responsible for the direct patients’ care following stem cell transplantation or cellular therapy treatment.
S/he will also provide administrative support to physicians within the Haematopoietic Stem Cell Transplantation (HSCT) and Cellular Therapy Program, assist the data manager with key clinical data at time points, and develop quality improvement activities such as care pathways for the transition care to other PHI partners.
Job Responsibilities
Primary Responsibilities and Duties
- Support multidisciplinary team members pertaining to patient care matters.
- Act as liaison officer, receive and coordinate new internal and external patient referrals.
- Conduct related and unrelated stem cell donor searches, facilitate donor identification process and communicate with team on the updates.
- Coordinate appointments for donors/patients throughout various phases across the continuum of stem cell transplantation care journey.
- Ensure all the appropriate investigations are ordered in the clinical system for all the different time points.
- Trace and follow up to ensure that all medical investigation results are generated timely.
- Prepare the necessary documents required for stem cell transplantation care.
- Assist patients and families to navigate across different services and locations.
- Provide patient and family education pertaining to stem cell transplantation care journey.
- Able to communicate effectively with internal and external stakeholders and provide smooth care transitions.
- Follow-up with patients through phone, where necessary.
- Perform internal documentation audit.
- Participation in Audit Preparation for the Haematopoietic Stem Cell Transplantation and Cellular Therapy Program.
- Be the secretariat for Haematopoietic Stem Cell Transplantation and Cellular Therapy Program. related meetings.
- Collect, update and maintain database.
- Participate in quality improvement activities.
- Identify areas for improvement to increase efficiency.

- Provide cross coverage to Clinical Research Coordinator (Cellular Therapy).
- Any other duties assigned by Reporting Officer.
